Nurse Navigator FIND Program-Remote Opportunity

This a Full Remote job, the offer is available from: Michigan (USA) Employment Type: Full timeShift: Description: POSITION PURPOSE Facilitates communication and coordinates care with the patient, primary and specialty care physician, and hospital departments on standardized care pathways. Ensures the availability, access, and timeliness of services to optimize quality of care, coordination, and patient experience. Provides evaluation, education, coordination, and referral to patients identified with an incidental radiological finding with recommended follow-up. Works collaboratively with other FIND/LCS staff. ESSENTIAL FUNCTIONS Knows, understands, incorporates and demonstrates the mission, vision and values of Trinity Health in leadership, behaviors, practices and decisions. Ensures all patients that are identified with incidental and/or unexpected findings with recommended follow-up are contacted and offered the option of the recommended follow-up. Provides personalized nurse navigation interventions to patients who request additional information, assessment and/or referral. This includes assessment of patient educational level, barriers to learning and other psychosocial factors that may impede remarkable care. Ensures provider time is utilized efficiently by ensuring appropriate follow-up intervals are recommended for incidental and/or unexpected findings and that duplicate follow-up requests are avoided. Supports provider decision making by: Directing patients to the correct provider based on the recommendation for follow-up of incidental and/or unexpected findings. Assisting PCPs in ensuring their patients get appropriate follow-up for incidental and/or unexpected findings at intervals specifically recommended by the radiologist. Obtaining additional imaging for incidental and/or unexpected findings only at the specific request of the PCP. Provides education and navigation through the follow-up process. Providing referral and education based on care deemed appropriate. Utilizes the available software programs to track daily/periodic assignments. Maintains frequent communication with local and regional colleagues involved in the Incidental Finding program as well as the Radiology and Oncology departments to facilitate the development of the program and maximize patient care. Functions independently while remaining calm in a high-stress environment. Maintains good rapport and cooperative relationships. Approaches conflict in a constructive manner. Helps to identify problems, offer solutions, and participate in their resolution. Performs other duties as assigned. Maintains a working knowledge of applicable Federal, State, and local laws and regulations, Trinity Health Integrity and Compliance Program and Code of Conduct, as well as other policies and procedures to ensure adherence in a manner that reflects honest, ethical, and professional behavior. MINIMUM QUALIFICATIONS Graduate of an accredited school of Nursing and Bachelor of Science Degree in Nursing. Three (3) to five (5) years of progressive clinical nursing experience is required. Current license to practice as a Registered Nurse in the State of Michigan. Additional state-specific licensing may also be required. Problem solving skills and ability to initiate creative solutions to problems. Demonstrated ability to expertly navigate a variety of software applications. Effectively communicate with Medical Staff, departmental staff, Trinity Health department representatives, physicians, other health providers and facilities. Working knowledge of Radiology procedures protocols and processes, preferred. Knowledge of Windows based programs. Effective organizational skills. Must possess a personal presence characterized by a sense of honesty, integrity, and caring with the ability to inspire and motivate others to promote the philosophy, mission, vision, goals, and values of Trinity Health. PHYSICAL AND MENTAL REQUIREMENTS AND WORKING CONDITIONS Must be able to adapt to frequently changing work priorities and be able to prioritize and balance the requirements of working with the System Office and Regional Health Ministries. Must possess good manual dexterity to operate a computer keyboard. Must possess good hearing to be effective on extensive telephone conference calls and for in person communication.
