Experienced Licensed Social Worker – Remote Medical Social Work and Case Management

Job Summary We're seeking an experienced Licensed Social Worker to join our team in a remote work-from-home role. As a Licensed Social Worker, you'll work collaboratively with a registered nurse case manager to assess, plan, and advocate for members' benefit plans and health needs. Your responsibilities will include researching available options and resources for members with financial, transportation, safety, and housing needs. This role requires an LSW/MSW with an unrestricted active license and 2+ years of experience in medical social work or case management.

Key Responsibilities

• Partner with a registered nurse case manager to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs
• Research available options and resources for members with financial, transportation, safety, and housing needs
• Empower members to make positive lifestyle and medical choices
• Work as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health
• Conduct telephonic or electronic exchanges with members to assess their needs and develop plans to meet those needs
• Collaborate with other departments to ensure seamless care coordination and communication Essential Qualifications
• Active and unrestricted LSW - Licensed Social Worker, LMSW - Licensed Masters Social Worker, LCSW - Licensed Clinical Social Worker, or LISW - Licensed Independent Social Worker in the state of residence
• 2+ years of experience in medical social work or case management
• Masters Degree in Social Work Preferred Qualifications
• Experience working with diverse populations, including children, adults, and families
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Experience with electronic medical records and other healthcare software Skills and Competencies

• Ability to work collaboratively with other healthcare professionals

Compensation, Perks, and Benefits

We offer a competitive salary range of $46,597.20 - $101,228.40, depending on experience and qualifications. In addition to your compensation, you'll enjoy a range of benefits, including
