Drug and Alcohol Case Management Specialist

Work from home Full-time role Hiring

About the position Adult D&A Case Management is a voluntary service designed to assist individuals in Allegheny County who have community-based needs and who also self-identify as having substance related struggles. Individuals can self-refer or be referred by any ancillary support they may also be engaging with. Once referred, individuals are screened and assessed to establish a psychosocial understanding of what their potential needs may be. As part of that comprehensive intake, individuals are also given the opportunity to develop a recovery plan with their case manager that outlines self-identified goals and the path by which they hope to accomplish them. From there, case managers will utilize harm reduction principles to support people through treatment processes, through court proceedings, in their pursuit of acquiring non-treatment related supports or even just in obtaining basic essential items. As part of this, Case Managers are tasked with muti-disciplinary cross system collaboration so that those who are being served are linked with all available resources in addition to having a solution focused experience with our services.
